Goal of the Cognitive Disability Strategy

1. DEVELOP AND INCREASE FAMILY, COMMUNITY AND PROFESSIONAL ABILITY TO SUPPORT INDIVIDUALS;

2. ASSIST INDIVIDUALS TO BECOME INDEPENDENT AND SELF-SUFFICIENT TO THE MAXIMUM OF THEIR CAPABILITIES; AND

3. PREVENT FASD BY REDUCING OR ELIMINATING DRINKING ALCOHOL DURING PREGNANCY THROUGH ACCESSIBLE, CULTURALLY APPROPRIATE SERVICES


CRITERIA FOR APPROVAL OF COGNITIVE DISABILITY

1. A case management team is in place.

2. A case plan is in place.

3. A case lead has been identified.

4. There is a cognitive disability (broadly defined).

5. There is an inability of agencies to stretch mandates further. Resources have been exhausted.

6. There are unmet needs identified by the team.

7. The individual is between 6-24 years of age.

8. Needs are confirmed & approval is recommended by the team.


Cognitive Disability is defined as:

  • Significant limitations in learning and processing information. Individuals are limited in retaining knowledge, learning skills, making decisions, communicating with others and;

  • Behaviour challenges which result in limited inter-personal, social and emotional functioning and;

  • Developmental challenges which limit capacity to adapt to daily living in areas such as self-care, independence at home, in the community, at work or leisure and;
  • Limitations and impairments that are persistent and long term and;

  • DSLA score between 1-5.
  • These individuals do not necessarily have to be complex cases or be at risk of harm to be included in the CDS. Individuals are also not included or excluded from the CDS on the basis of any specific diagnosis.

  • - Cognitive Disability Consultants
    - Individualized and Flexible Supports including funding
  • Improving Access to Assessments and Diagnoses and to Case Teams

  • - Enhancing the multidisciplinary teams and training
  • Strengthening Prevention and Intervention of Fetal Alcohol Spectrum Disorder
    - Training for front line service providers, parents, community
